Personalized Health Improvement Strategies through Evidence-Based Medicine

The Rx Ohio Collaborative calls for ensuring clinical efficacy through programs and strategies based on  evidence-based research. After clinical considerations are accounted for, we have a single aim: driving out wasteful spending in the benefit.

With the introduction of the Institute of Pharmaceutical Outcomes (IPO) in 2009, the RxOC now truly differentiates itself from all other pharmacy programs offered to public-sector employers. Led by The Ohio State University Health Plan Inc., the IPO has pulled an independent team of clinical advisors, researchers and staff to provide proactive and unbiased results and recommendations on pharmaceutical issues facing Ohioans. The Ohio State University Health Plan Inc. and Express Scripts serve as the IPO Core and Support Staff while the OSU Medical Center and College of Pharmacy along with other universities in Ohio serve as the expert panel. Institute of Pharmaceutical Outcomes’ goal is to identify benefit design opportunities and incorporate scientific evidence and claims data to then develop actionable steps for members.

Population Based Health Management through Innovation

Medication Therapy Management

The RxOC has pledged significant resources toward the development of innovative programs to improve healthcare delivery in Ohio. First among these initiatives is a new Medication Therapy Management (MTM) program being developed at OSU, which the RxOC began as a pilot in 2009. This personalized MTM program grew by patient demand from a small-scale MTM service offered. It is a face-to-face or telephone appointment with a University pharmacist, who knows the patient’s benefit plan and can recommend and advocate for optimal drug choices for the patient. The pharmacist can, if desired by the patient, contact the patient’s physician’s on the spot to recommend medication changes. The pharmacist will report to the patient’s physicians about the consultation and will provide the physician with independent clinical judgment on the best medication for this particular patient. This high level of personalized medication management, in conjunction with education of both patient and physician, contribute to the best overall health outcomes and cost savings while managing medication therapy and safety.

Population Health Management/IntellActSM Focus on Data Integration

Express Scripts implemented a best-in-class program integrating medical, pharmacy, lab and self-reported data to give prescribers a 360-degree view of patients for improved decision making and for the client, improving the health of patients and cost effectiveness of their overall spend. This program began as a pilot in 2009 and focuses on prevention of drug safety issues, addresses gaps in care, and assists in maximizing adherence. Using a best-in-class rules engine, alerts are generated to prescribers retrospectively. This program enables better overall health and value.
